As Sacred Heart Parish moves forward with the planning of our Faith and Wellness Mental Health Ministry, we are look for your feedback to help us to better understand the interest and needs of our parish community. We appreciate your participation.
The survey is attached in this post. Currently, you can print it, fill it out manually and return it to the Parish Office Monday-Friday between 8AM and 5PM. We are working on creating the survey in Google Forms to allow you to fill it out and submit it electronically. In addition, hard copy surveys will be available at the church entrances on the weekends of November 7-8 and November 14-15.
